TERESA JEAN GOTZ, age 56, of Columbia City, IN passed away at 5:55 p.m. Saturday, July 6, 2013 at Visiting Nurse and Hospice Home in Fort Wayne. Born on February 26, 1957 in Enid, OK, she was the daughter of Russel B. and Jacqueline June (Spires) Gotz.

She had lived in Whitley and Allen Counties. She graduated from Columbia City High School with the Class of 1975 and Manchester College with a BS in 1981, Parkview Methodist School of Nursing in 1984 and a Masters of Social Work from IUPUI in 1991.

She worked at Charter Beacon and for Dr. Rustagi both of Fort Wayne, Parkview Whitley Hospital in Columbia City and Lutheran Hospital for almost 6 years in the CIC Unit.

She was a member of Columbia City United Methodist Church and was on the committee for "The Art of Nursing: Healing Hands, Trusted Care" book produced by Parkview. Her main focus was her patients and her family, especially her nieces.

Survivors include her sister, Michele (Gary) Wolff of Wichita, KS; brother, Jon R. Gotz of Columbia City; and nieces, Theresa and Larisa Wolff both of Wichita, KS. She was preceded in death by her parents, Russel and Jacqueline Gotz.

Visitation will be held Thursday, July 11, 2013, from 2 to 4 and 6 to 8 p.m. at DeMoney-Grimes Countryside Park Funeral Home, 600 Countryside Drive. Funeral services will be held at 10:30 a.m. Friday, July 12, 2013 at Columbia City United Methodist Church, 605 Forest Pkwy. with calling one hour prior. Rev. David Arnold officiating. Burial will follow at South Park Original Cemetery.

Memorial gifts may be given in Ms. Gotz's memory to Whitley County Humane Society, Columbia City United Methodist Church or Visiting Nurse & Hospice Home.
